- Application - Responsible Declaration according to article 8 of Law 1599/86, where the interested party should express the following:
a. I have the usual residence in Greece.
b. I have a valid Greek or EU member state driver's license. (including the states of Norway, Iceland, Liechtenstein) and has not been removed by any authority.
c. I do not have a driver's license from a third country.
d. I express my wish to have my Blood Type recorded on the driver's license document and I agree to its storage in the Ministry of Infrastructure and Transport's Driver's License Record, a document entered on page 2 as point 14a of the Driver's License.
- Photocopy of police ID or current Greek passport. In case the applicant is a citizen of another EU member state, he is requested to submit a photocopy of his identity card or current passport, while for a citizen of a third country, a photocopy of his passport or other document allowing his entry into Greece, issued by a competent Greek authority, is required origin and from which identity and age derive.
- Photocopy of any valid public document certifying habitual residence in Greece, such as Residence Permit, Certificate of Registration of an EU Citizen, Certificate of Permanent Residence of a Citizen of an EU Member State, Identity Card (with reference to the residence permit position), Certificate of filing supporting documents for granting or renewing a Type A Residence Permit, Special Homogeneous Identity Card. This document must have been issued at least 185 days prior to the submission of the application. In case the applicant is a foreign student or student, the original certificate from the institution or school is required, confirming at least six (6) months of student or student status prior to the submission of the application.
- Photocopy of a valid driver's license or the license he holds, if it has expired. When submitting the documents, the applicant is requested to present all the driving licenses he holds (car, motorcycle, or moped, which was issued by the Traffic Services of the Greek Police) for the issuance of a new single license.
- In order to obtain a driver's license, two health certificates are required, one from an ophthalmologist and one from a pathologist. For the issuance of the certificate, a fee must be paid to each physician who performs the examination, which is equal to the minimum minimum fee established from time to time for physicians contracted with the State. In addition, the presentation of a health certificate is required. In case there is no health book, laboratory tests such as general blood test, sugar measurement, urea test, cardiogram, and chest x-ray are required.

- For driver's license holders over 80 years old, in addition to the previous certificates, two additional health certificates are required from an otolaryngologist and a neurologist (or neuropsychiatrist or psychiatrist), doctors contracted with the Directorates of Transport & Communications of the Attica Region.
- In the case of a hard of hearing or deaf mute, in addition to the pathologist's and ophthalmologist's certificates, a certificate from an otolaryngologist, contracted with the Transport & Communications Directorates of the Attica Region, is also required.
- For driver's license holders under 80 years of age, who have already been examined by a Secondary Medical Committee in the past, certificates from doctors of the specialties that made up the D.I.E. are required. from which they had been examined.
- Health certificates are valid for 6 months from the date of issue. All of the above procedures are issued as a single receipt (e-receipt code "39").
- At least one recent passport-size photograph must be submitted to the Department of Transport and Communications.
- A driver's license print application is required.
