- Application – Declaration of responsibility in accordance with Law 1599/86, in which the candidate expresses the following will:
a. She states that she is used to residing in Greece, either as a student or as a student for at least six (6) months prior to submitting the application and related documents.
b. He declares that the driver's license, for which he is requesting a copy, has been lost or stolen.
c. He confirms that he does not hold another driver's license, either Greek or from a member state of the European Union, nor from the states of Norway, Iceland, Liechtenstein, nor has it been revoked by any authority.
d. Certifies that his/her driver's license is not withheld by any authority, whether Greek or by a member state of the European Union, nor by the states of Norway, Iceland, Liechtenstein, for any reason.e. Expresses his/her desire for his/her blood type to be recorded on the driver's license form and consents to its entry in the Driver's License File of the Ministry of Infrastructure and Transport, on page 2, as point 14a of the Driver's License form.
- Photocopy of identity card or valid Greek passport. In case the applicant is a citizen of another member state of the European Union, he must submit a photocopy of an identity card or a valid passport, while for a citizen of a third country a photocopy of a passport or other document allowing his entry into Greece is required, or a document from a competent Greek authority that establishes his identity and age.
- Photocopy of a valid public document, if the person concerned is not a Greek citizen, confirming his or her habitual residence in Greece, such as a residence permit, EU citizen registration certificate, document certifying permanent residence of a citizen of an EU member state, etiquette ID indicating the location of the residence permit , certificate of submission of supporting documents for granting or renewing a type A residence permit, special expatriate identity card. The above document must have been issued at least 185 days before the application is submitted and, in case these are not complete, two additional pieces of evidence are submitted, such as tax awareness, lease or home ownership contract, social security document, children's education certificate , energy or telephone or water bill etc. In case the applicant is a foreign student or student, the original certificate from the educational institution itself is required, confirming the six-month period of study before submitting the application.
- A photocopy of a valid driver's license, even if it is damaged or defaced.
- Proof of payment for the printing of the driver's license (e-receipt code "28").
- Proof of payment of fixed stamp duty and levy in favor of third parties (e-receipt code "66").
- Proof of payment in the event of a driver's license copy being issued due to loss or theft (e-receipt code "29").
- One (1) recent passport-standard photograph.
- Application to print the driver's license.
