- Application – Declaration in accordance with article 8 of Law 1599/86, where the interested party states the following:
a. Has habitual residence in Greece or has either the status of a student or student for at least six (6) consecutive months prior to the submission of the application and related documents.
b. He does not hold another driver's license, either Greek, or from a member state of the European Union, or from the states of Norway, Iceland, Liechtenstein, except for the license he requests to be replaced, which has not been revoked by any competent authority.
c. He does not hold a driver's license from a third country.
d. Expresses the desire to have his blood type included in his driver's license and agrees to the entry of the relevant data in the Ministry of Infrastructure and Transport's Driver's License Record, recorded on page 2, point 14a.
- Photocopy of police ID or valid Greek passport. In the event that the applicant is a citizen of another European Union member state, he submits a photocopy of his identity card or valid passport. Accordingly, for a citizen of a third country, a photocopy of a passport or other document allowing his entry into Greece, issued by the competent Greek authority, with a clear reference to his identity and age is required.
- A copy of any valid public document is necessary, as long as the person concerned is not a Greek citizen, and certifies his habitual residence in Greece. The following documents are included in this category: Residence Permit, Certificate of Registration of an EU Citizen, Certificate of Permanent Residence of a citizen of an EU member state, Etiquette ID that records the residence permit position, Certificate of submission of supporting documents for the grant or renewal Type A Residence Permit, Special Identity Card for Homogeneous. This document must have been issued at least 185 days prior to the submission of the application. In case these conditions are not met, two additional pieces of evidence are submitted, such as proof of taxation in the country, proof of ownership or rental of a residence, proof of social security, proof of children's education, energy or telephone or water bill, etc.
- In relation to foreign nationals who are students or pupils, the submission of the original certificate from the school or university confirming the minimum six months of student or pupil status before the day of application is required.
- Also, a copy of the valid driver's license or licenses (car and motorcycle if any) is required, as well as the corresponding moped driver's license, as long as it was issued until 1/18/2013 by the Traffic Services. In case of loss or theft of the moped driver's license, an additional certificate from a Security Service confirming the loss or theft is required.
- Proof of payment is also required to print the driver's license with the e-receipt code "28". Finally, a recent photograph, in accordance with the passport type, must be submitted. In addition, the request to print the driver's license and, in case of damage to the license, proof of fixed stamp duty and third-party contribution with the e-receipt code "66" are required. In case of correcting the date of birth, a recent original birth certificate is required, while correcting a name or surname requires an irrevocable court decision, divorce certificate or a certificate from the competent municipality confirming the identity of the driver's license holder.
